Depending on the type of visit, you may receive forms, questionnaires, consent documents, lab instructions, or other information before your appointment. Completing these ahead of time allows our clinical team to spend more of your appointment focused on you.

For aesthetic and laser appointments, you may receive treatment-specific preparation instructions before your visit.
